Your card was declined multiple times for your $9.95 monthly fee. You account was thereby cancelled. You then chose to reactive your account. The reactivation fee is $9.95 plus the past due $9.95 monthly charge. Thus your card was charged $19.90.
When porting a number from another carrier the information must match exactly or the port will be rejected. This basically starts the process over again. If everything is accepted on the first try, ports tend to happen within about a week of their request.
